Jermain was clearly wacked out and his eyes were doing the hula hoop. Pavlik didn't get stopped because he's used to rolling around town in daily life while being knocked out by liquor.
Smoger revealed in an interview why he didn't stop it in the second. These guys, at least Smoger, go to seminars to continually update their knowledge of the sport and rules. Smoger said he knew from one of his seminars that Kelly had taken an "equilibrium shot" behind the ear and therefore had a chance to recover. I'm guessing there are a lot of clues that they look for that only they would see up close that we miss. The eyes being the most obvious.

The TS should watch the slo-mo. JT's troubles started with a perfectly executed 1-2 mid-ring. He went back to the ropes and then slid over away from Pav's right hand. Pavlik moved with him and trapped him in the corner. It looked like JT might have a chance to punch his way out of trouble until KP hit him with a perfectly executed uppercut/left hook. From that point on, Smoger could have counted to a hundred, and it would not have made any difference.
Jack Loew asked Kelly, "When did you know you had him". Kelly resonded that it was that mid-ring exchange because he saw one of Taylor's chest muscles twitch. I've seen a slo-mo replay of that. Basically that punch was enough to disrupt Taylor's nervous system.
