    Theres no contradiction. He stated he had no injury and therefore couldn't get his happy shot. If he just wrote down YES this wouldn't be happening and claiming sabatoge is just ignorant, juvenile, or blatant trolling

    How do you not see a contraction. Do you know what a contradiction is? Dude, it's plain as the day. If you don't then you're either in denial or psychotic because there is a CLEAR contradiction in the med form.

    You don't make no for injury and then mark down 5 different serious medications (including cortisone!) without there being some sort of problem.

    If you were taking all of those meds why would you mark down NO INJURY?

    So, you answer it for me. Why would he mark down all those meds if he also marked down no injury? Obviously a mistake somewhere, like you suggest.
