Nandrolone can occur naturally in body in tiny amounts. They test the urine for nandrolone metabolites above 2 ng per ml of urine, as this is considered the maximum amount it could occur natUrally. Of course it can also be synthesised.

 

In fairness, I did a search and the literature would appear to support that false positives can occur BUT most athletes who managed to weasel out of a supposedly false-positive then admitted it further down the line that they took it. The chances him and his cousin both testing + for the same anabolic as a false positive is rather low.
