A retina tear is something clear cut. It will show clearly on any scan. It requires surgery literally within days or a week or so at the most. Otherwise it risks becoming fully detached which is much worse.

I'd consider it something which is hard to fake.

Spence's training camp was **** poor. No content pushed out, very few interviews, and the leaked sparring footage of Spence getting pummeled by a small southpaw. The footage doesn't really mean anything, but when you combine it with the fact that there was next to no footage, and no interviews, and basically no hype or words from the Spence camp, you can draw three conclusion that **** was bad.
