An elbow caused the cut, then a bite ripped it open.

what was worse was between rounds Joe Souza the cutman gouged that eye with the towel and ripped it open even worse you can clearly see this on the HBO version. I know he did not do it on purpose but it literally ripped it open even more. It was nasty.

Souza was very good he just put too much pressure and raked downward on the cut I am not sure he knew where it was because of all the blood.
