Martinez left hand against Paul Williams is a great example.

Froch throwing a wide left hook at George Groves that he would block easily but as he was blocking the straight right was on its way already.
Another common one is when a fighter jabs to the body for a few rounds and then feints the jab to the body and comes up with a left hook(Maidana to Broner) or keeps dipping and throws an over hand right(Martinez to Williams) there are many other examples where the jab to the stomach is a set up/diversion punch

    Tyson vs Ruddock 1.

Tyson sticks his chin out and lets Ruddock hit him with 3 left smashes, meanwhiles Tysons loading up a massive left hook which sends Ruddock on his ass.

A big risky trap, but trap nonetheless.

    Also, Froch vs Groves 2.

Groves gets ready for a left hook, Froch steps to the right, throws a 1-2 and Groves goes to sleep

Lewis vs Tua. In the last few seconds of the last round, Lewis waits in the corner (ala Mayweather/Hatton), Tua jumps forward with a left hook only to land head first into the ring post

    1) Floyd's check hook against Hatton.
    2) Marquez's full body right hand counter against Pac in their fourth fight
    3) Pac's second KD against Cotto, forcing him into a brawl, taking advantage of the opening by landing an uppercut from an angle Cotto didn't expect
    4) Hopkins vs Trinidad where he intentionally telegraphed an uppercut to make Tito throw the left hook, then blocking it then countering over it with a right hook that sent Tito down
    5) Numerous examples of Floyd's pull counter
    6) Pacquiao vs Marquez 2, where Marquez committed to a right hand that missed, and tried to follow it up with a left hand, but it left him way open for a short and fast left hand counter from Pac that knocked him down.

Knockdowns of Thornton, Sosa and Griffin (2nd fight, KO) all come to mind.

A good trap is set by repeating the same thing over and over.

I think a good example that sticks hard is Maidana setting up his first knockdown on Broner.

Broner had been getting used to maidanas jabs to the body. The jab to the body put broners hand in a position to parry it. In this position it was far away from his face.

Broner, who began to Parry the jab without thinking too much saw maidana bend down like he was going to throw a jab, only to come up with a hook. Broner's hand was still down and it found a home on his Glass mandible.

EDIT: Found video. Paulie explains it.

Marquez continously pounding Pacquiao's belly with right hands in fight 4, then in the 3rd round, Marquez dips low as if to go to the belly again, but instead goes over the top with a roundhouse monster right hand that floors and almost takes Pacquiao's head off.

Pacquiao was dropping his hands to defend thinking Marquez was going to his belly again. Marquez disguised going low but instead went over the top.
    Pacquiao fell for the trap.

Off that, Maidana baiting the pull counter and countering it was actually one of the coolest things I've seen anyone do against Mayweather.
