In all seriousness, I believe he died in a bar brawl. The circumstances of which seem to be a bit hazy.
You were hoping for some dramatic story like how his brains got scrambled by Wlad and how he died after being in a coma for two weeks

His fight with Ed Mahone was awesome!!! (The video of which would truly entertain) Cody actually was a tough man fighter turned boxer. Cody Koch, seems more talented then alot of the heavyweight contenders today. Looks like he progressed alot from the toughman to his last fight. Outside of Wlady, he looked good until he got caught by Ed Mahone.
