Sharkey looks genuinely in distress there obviously from the body shot rather than the left hook to the jaw. Was it ever established if the blow was low, as I seem to remember they were in a clinch and Dempsey had his back to the camera?
The body blow was not deemed a low blow by the referee. It was an accumulation of body blows by the best body punching heavyweight ever, that took the wind out of Sharkey leading to that left hook. Sharkey always claimed that Dempsey was a paralyzing puncher who could "break bones wherever he hit you..."

Can't see the drama, he had no problem with his nuts right before the left hook. If your nuts have been truly hurt you are sure as hell doubling over in pain. There's not much worse.
