Good work Reznick, for someone who has been boxing for just four months this is good. A couple of things I noticed that you might want to think about is varying your movement when punching the bag, I think you circle the whole time clock-wise, and the movement is very slow and deliberate, I would work on this. Also get a bit more movement when striking the bag, go low with straight jabs and cross to the body and then come up stairs with a hook or two for power. Also watch how you deliver the right hook, a notoriously difficult punch to deliver well in practice - you seem to drop your left and leave yourself open. Throw in a few more uppercuts, and really swing those hips. Like I said - good progress for the amount of time
Nice man, Keep working you'll only get better. Bag work is key for boxing conditioning, if you ask me its one of the best workouts period. When you start competing make sure to have days where you put in solid 6-10 on the bag you'll notice when you spar the improvements especially in stamina. Another thing with bag work that I'll note, try to only work in serious work on the bag 2-3 times a week or you'll wear yourself out. I learned to go hard on the bag work all the way up to a week out from the fight then stop all bag work completely and you'll feel great once you step in the ring.

You have a good start for 4 months. I'd advice you to start to learn to be a little more defencive boxer. Imagine the heavybag as a real opponet and react by taking your hands to high guard / blocking punches, slipping etc. For me defence is the most important trait as a boxer. So remember to always be prepared to block punches and slip.
You're absolutely right. A "cool down" is no excuse to stop using head movement, or it could become a bad habit. I need to get more disciplined with that.
