Still looks to me like Carnera hit him with the inside of his forearm. Another camera angle would have helped. What was the consensus at ringside?
Just rewatched the KO part. I can see why it would look like he hit him with his forearm, but Primo probably could KO a guy with just his forearm.

It's not just what he hit him with, Rob. It's the form of the punch. He is swinging his left arm out and following it with his body. Nothing arrests the motion to give the uppercut snap. He isn't bending into it. He actually swings both arms together like he's a rock star throwing a bucket of water over the front rows. I just don't see how it could have been devastating. It's in link below at about 8.30 (8.30 - best ever line in a Laurel and Hardy film. Blockheads where a ball comes down a flight of stairs and hits a clerk in the face - but I digress). Dempsey was walking forward, threw a left that had no push coming through from bended legs, no twist of the hip, no snapping of the shoulder or elbow, just a straight clubbing blow. Sharkey took it on the cheek and threw himself forwards. Preposterous. Apologists say Dempsey was so powerful he could knock you out with a punch that was thrown that poorly. No, he couldn't, otherwise he would have won every fight by early kayo. 9.50 below if anyone is interested. McCall lost to Orlin Norris who was a CW and he wasn't anything special. Sharkey and Schmeling would school McCall. It's not that hard to beat McCall, he's very tough and heavy handed but nothing suggest that he was better than just a weaker contender.
