Doesn't Cotto balloon up to almost 170 nowadays?

Nope... although they're often parallel, it isn't the same thing.
For example, Ward is the current lineal SMW champ, he doesn't have the Ring belt anymore because he was stripped of that quite a while ago. If Ward's next fight would be at SMW, it still would be for the lineal title.

Often a new lineage starts with winning the Ring belt, but who really can say where some start and end? There's a big debate about Wlad's "lineage" for example.
Also lineal can't be stripped, the Ring belt can.

Makes sense. I guess It's almost impossible to exactly define how the lineage works.

it's all up on interpretation, so not really very significant at all.
It's very possible that within a couple of months Golovkin will become the "undisputed" Middleweight champion, if either Cotto wins or Canelo wins and vacates and GGG fights Lee/BJS next.
Then he won't be lineal but he will be undisputed, just like Tyson back in the day.

IIRC the cruiserweight limit at the time was 190lb- so back then Jones was a heavyweight.

There's been over times a fighter has weighed below the bottom end of the weight class though.
