Do you actually think Pacquiao's first comment was refering to a 'fear' of tattoo needles? Obviously, he was refering to a 'fear' of medical needles (syringes), so the video adds consistency.

And even you have to admit (besides both being called 'needles') there is little similarity between getting a tattoo and having blood drawn (or being injected) with a medical syringe. For example, cultures where tattooing is considered a sign of strength would not consider having your blood taken as sign of strength. Do you know why? Because they actually realize they are two completely DIFFERENT things.
