Prior to that particular punch, Cotto attempted a left hook to the body that Foreman was able to pick off. So he feinted with his right glove to get Foreman to get commit to one side, and then followed it up with the body shot right underneath Foreman's right elbow. It looks like a sweeping left hook to me, but I guess maybe you can call it a hybrid left hook/uppercut.
If you look, his good knee buckles and dips after the shot, and his injured knee is actually left supporting him as he crouches. So clearly, as RJJ said as it happened, that signature hook put him down. Not exactly hard to believe.
