Not quite the same, but I know one example of two fighters TKOing each other at the same time: 'Prince' Charles Williams against Merqui Sosa in 1995. After 7 rounds the ringside doctor declared that both men had rendered the other one unable to continue because of the heavy punishment they'd both taken (both were badly cut a couple of times over, I recall) and therefore both of them were technically stopped, and the fight was ruled a draw. Lee Roy Murphy and Chisanda Mutti fought an unbelievable Cruiserweight title contest (check it out if you haven't seen it) back in the 80s. In the twelfth round both of them landed big shots at the same time and went down, and the referee gave them both a count simultaneously. But Murphy got up in time, whereas Mutti couldn't. So not quite a double knockout.
