Do you respect fighters who are not prepared to die in the ring?

True, and to be fair his toe did actually look really sore. However, This was for possibly the biggest prize in sport, the Heavyweight Championship of the World. To win this prestigious title, a fighter needs to be prepared to put something on the line. I don't believe they should be prepared to die (it's only a sport), but they shoiuld at least be prepared to be hurt and humiliated.

If Haye wasn't prepared to risk getting knocked out, and didn't want to push through the pain barrier of a sore little toe, then unfortunately he is not made of the stuff of champions, and loses a lot of respect from me.
Take a look at what other fighters have gone through, for supposedly lesser titles:

Arther Abrahams suffered a very nasty broken jaw for most of his fight against Miranda, yet fought through it, and won.

Danny Williams fought on with an awful dislocated shoulder against Mark Potter for the British title, and got his rewards with a knockout win.

Danny Williams (again) still trying deperately to knock out Vitali despite knowing he was going to keep getting knocked down.

Even Audley Harrison showed more bottle than Haye in the rematch with Sprott, fought most of the fight one handed and got rewarded for his efforts with the KO.

I could go on, but you get the jist. I always thought Haye had a bit more to him than what he showed against Wlad. He had balls at cruiserweight, but I actually believe Adam Booth had them chopped off when he moved up to heavyweight.

The strength of the injury has no bearing with that kind of bravery.
