theres various different things and charecteristics that make a puncher, one thing to consider is fist size you can bet pauli malignaggi does not have big fists , you can bet maidana does.
i know what you mean, i use to know this skinny, lanky guy whose arms werent big at all, but he could punch like a ton of bricks, probably has something to do with the tendons/muscle fibers in your arms, some people just naturally have stronger tendons and muscle fibers regardless of how big their arms are in size, just like someone who never trains at running, but is just naturally super fast
Its comes natually to some, but it's down to good technique. Legs and core and the explosive whirl of the shoulder and back muscles especially. And strong forearms, wrists and hands clenching at impact. Good wrist size. Forward body motion. Stepping with the punch. Timing the punch as your opponent's coming in. Punching through the target like you mean it. And Bad Intentions.

Natural ko power if by that you mean punching power with the least effort possible or least energy then mass would be the biggest factor. You can be much more effective with technique. Short, compact, precise punches can be more effective than hayemakers. A good example is Morales vs Maidana. The generalization is you need to be an excellent physical specimen with huge wrists and calves but I disagree. Morales was every bit as devastating as Hamed. Hamed I do think was a natural puncher while Morales a classically trained one. His decapitation of Angel Chacon one of the most frightning kos ever at least from the angle I saw it. Now if Hamed the naturally gifted fighter didnt rely so much on his talent and instead learned some technique like Manny did he would be enjoying similar success. But yea basically mass x velocity=force.

some guys are just genetically gifted. i can easily relate to this, because when i was young i was able to run and jump higher than my peers. I was physically stronger, ie strenght to weight ratio is higher. how many 5'8" guys can jump and reach that 10 ft ring in basketball? unless you are spud webb...
