Hopkins won all 4 major titles in seperate fights. Jermain Taylor gave up the WBA and IBF belts so he could dump the mandatories, rematch Hopkins and fight Winky. I have no idea why Martinez let the WBO title go. All he had to do was commit to defending it. It doesn't really matter. He's the lineal champ. It goes Hopkins, Taylor, Pavlik, Martinez. Anybody who is not part of that lineage is just a titlist, not the champ, and the middleweight division is still the only division in boxing with a direct and easily traceable lineage back to all 4 major titles.

DM, Calzaghe & Hamed beat the holders of all 4 belts. Bowe won the wbo title 2 years after losing the others.
there would be many more "Undisputed" champions if this was not a factor... quite a few relatively current "unknowns" hold a titlebelt, and cling onto it and avoid risky fights... they hold onto the belts to build up their own local legacy
Correct. Unifying all four belts really isn't a big thing, or not as big as it's made out to be. It just means the fighter needs to face whatever shitty mandatories the ABCs put in front of them instead of taking fights to actually make money.
In Calzaghe's case though, he vacated the IBF belt before the Manfredo fight, not before the Kessler fight. Joe decided to make a meaningless defense against the overmatched Manfredo instead of making a mando defense against the overmatched Stegieltz(who's now the WBO champ).
