Mike Tyson defended the title every couple of months in the 80's.

Ali defended the title twice in the space of 25 days in 1976.

What's the restriction on championship defences now and when where they introduced?

I don't think there is one? Just a matter of how quickly they can get back in the ring against a top 15 ranked fighter

Individual jurisdictions have rules about how often you can fight (I think 1 month between fights is pretty common) but that only applies to the jurisdiction. You could fight in Nevada and then fight next week in Mexico if you don't care about your health.

Perhaps the sanctioning organization has a rule? I'd be surprised if it was much longer than a month.

Their shouldn't be a problem with numerous defenses as long as you're medically cleared.

You have to be medically cleared which includes cuts, bumps, bruises and fractures from the previous fight. If you were concussed this has a minimum time period, can't remember because of all the concussions :D. You can obviously cover up a lot of stuff especially in certain places and at lower level, but at the highest level I'd say you can't cover up jack and as others said if you care about your health. The further back you go, the less restrictions. To answer the question if you were taking little damage and getting quick wins as champion you could fight frequent enough, then you have promoters etc ruining things, who would rather you fight once a year if it meant more 0's on their paycheck.
