Konck means that Pacquiao is not psychologically comfortable of a needle being injected and taking blood out of him. There is difference between putting a tatoo on your biceps, and blood being taken out by injecting a needle on your wrist.

Even I is more unease with injecting a needle on my wrist and taking some blood, than putting a tatoo on me.

Look dickweed, if he was really afraid of any needles then he wouldn't agreed to do any testing whatsoever. He is superstious given that the last time he got blood drawn not long before the fight he lost. And yeah, getting a tattoo when he's not fighting is the same as giving blood on the day of the fight.
