Hello, I'm trying to tailor a routine for myself to follow.

I did almost nothing for the last six month and to say that I am out of shape would be an understatement. I run two miles in about 23 minutes.

Last time I sparred, I gassed out after two rounds (I could do 4-5 rounds 1 or 2 years ago...). Roughly, Two years ago, my coach told me I needed lots of sparring at this point so I know in terms of boxing, that's roughly the level I'm at (didn't do much since then).

What's the most intelligent way I can go about it? It seems to me that every beginner program assumes a certain level of fitness (which is fair, as most interested in amateur boxing seems be athletes to begin with, but that is not my case at all).

There's no magical recipe here. Simply be consistent and realistic with the goals you set yourself. If you know you're badly out of shape take your time and don't over exert yourself. As long as your consistent and making improvements week after week month after month you'll reap the results. In addition your diet needs to be on point.
Sometimes its worth working out on your own as at least you can set your own pace if you're finding the classes too difficult. As long as your consistent, have a good diet, and can up your intensity gradually you'll see results in less than a few weeks.
