There's nothing taken out of context about it. He is referring to Pac, and that his health comes first when it comes to fighting that man, implying that Pac is bad for his health, more so than any other fighter cos this health talk certainly didn't come up prior to Pac.

Rich coward is also not out of context. He says he is a coward, but at least he's rich. That's like me saying "maybe I'm a homo***ual, but at least I'm a rich homo" - still homo. He alludes to the fact that he doesn't care whether the fans think he's a coward or not because he's rich, meaning he knows fans thinks he's a coward, acknowledging hes a coward, but he got money so he doesn't care.

i have not seen the video in ages (which is funny because of how much its posted by idiots) but he clearly implies if pac wont take the tests that he aient going to fight him as the tests are there to gaurentee both fighters are clean and on a level playing field, health comes first in this regard means he aient going to risk his health and career fighting someone who ran there own name through the mud by coming up with bs reasons to not partake in blood testing for banned substances. people might call him a coward but he will be a rich coward.

And sure he can hide behind the test, cos we know what happened when Pac agreed to the tests, he found a new excuse.
