Ali had a longer period of inactivity after winning his belt. Would you rather Wlad v Fury face off when Fury isn't fully fit? Have a repeat of the Lewis V McCall incident? Whether or not Fury has his belts, everyone knows he's the real HW champion.
Yet Ali was stripped of the WBA title during this period, so based on your example why shouldn't the WBA now do the same with Fury? Fact is Fury has twice been asked to provide medical proof of his mental illness and has to my knowledge not done so. If he cannot provide a medical certificate for his illness he should be stripped. As for being the true heavyweight champion, with or without the belts, that will be ultimately decided by his NADP hearing for his doping. If his appeal fails and the charges brought against him by UKAD are upheld then he technically was never the real heavyweight champion.
