Well, Cotto never beat the best at 154 but he was ranked by some to be the best(back then the division was rather bleak). At 160 he beat the champion.
"Some" is a key word. Overwhelming majority didn't rank him the best at 154 and certainly doesn't rank him the best at 160.
From Foreman till Mayweather he was number 1 by most publications. Now he's number 1 purely on paper.

You do realze Foreman was the #1 rated JMW Champ when he fought Cotto & Martinez was the only real MW Champ as well. A 10% Martinez was supposed to be able to beat Cotto according to fans, and Manny Pacquiao ducked Foreman due to styles just before Cotto fought him. Up until the Mayweather fight everybody rated Cotto as #1 at JMW just like the overwhelming majority consider Cotto to be the true MW Champion even if they dont see him as the head to head best.
You make some good points, what I mean about the land of make believe is that things are how they are not how we wish them to be. For there to exist an actual sport you need organizations that will make and enforce rules. And people have to agree to follow said rules. And yes they are bent and broken all the times this is the same thing that happens in business and government. Any man made institution can and will most likely be corrupted. That is life.
Cotto could've just paid the sanctioning fee for this fight like he did for the Martinez and Geale fights. He would be defending his title Saturday. If he won he would actually have to fight GGG next, so the sanctioning fee is a problem now.
