Is it fair to assume that this phrase originated from the time when in many boxing jurisdictions title fights that did not end in a ko were labeled "no decisions"?

In part. However, a draw goes to the champion, so we officially recognize that the champ has the benefit of the doubt. If we did not recognize the principle that you have to take the title from the champ, then a draw would leave the title vacant.
