I guess he idolized McCullough growing up, no surprise - and went up to shake hands with him in the hotel lobby before a presser for their fight. Pocket Rocket didn't even realize Fenix was his opponent and blew him off - though he'd later claim he didn't even see or hear Ruiz approaching him. Ruiz was convinced it was a slight. Given that he pounded the old man into retirement, I'm guessing he brought the fury.
